Restoring a Receding Hairline with a Hair Transplant

For most men, recession at the temples is the first visible sign of hereditary hair loss and often what bothers them most when they look in the mirror. Recession along the forehead and temples can quickly make the face look older, even when the rest of the hair is still full.

If you are a suitable candidate, a hair transplant can give fuller coverage in the areas where your hairline has receded and create a natural-looking hairline.

At Elithair, follicles are taken from the donor area of your scalp and transplanted along your planned hairline. These follicles are generally more resistant to androgen-related miniaturisation and can last long term.

Before and After: Hair Transplant for a Receding Hairline

A receding hairline is one of the most common reasons patients come to Elithair. The hairline is the most demanding area to treat, so careful planning is needed to achieve a natural-looking result (Shapiro 2004).

What Is a Receding Hairline and What Causes It?

A receding hairline develops when the hairline gradually moves back at both temples. The hair in the centre usually stays dense at first, creating the typical M or V shape that many men begin to notice in their mid-twenties.

In medical terminology, this frontal recession is also called calvities frontalis. For most men, recession at the temples is the first visible sign of androgenetic alopecia, or hereditary hair loss, which affects up to 80% of men during their lifetime (Kanti et al. 2018).

Genes and hormones both play a role (Lolli et al. 2017). The enzyme 5-alpha reductase converts some of your testosterone into the hormone DHT (dihydrotestosterone).

In men with an inherited sensitivity to DHT, follicles along the forehead and at the temples react particularly strongly. DHT shortens their growth phase, and the follicles shrink with each cycle, a process called miniaturisation. Hair that was once thick grows back thinner, shorter and lighter until it stops growing altogether. This inherited sensitivity means that receding hairlines tend to run in families.

It is important to distinguish a receding hairline from a mature hairline. Between the ages of 17 and 27, many men's hairlines naturally move back by around one to one and a half centimetres and then stabilise. This is a normal maturation process rather than pathological hair loss and does not in itself require treatment.

Comparing photos over three to six months, alongside trichoscopy performed by a dermatologist, can show whether the follicles are progressively shrinking or the hairline is simply settling into a mature position.

Norwood scale

The Norwood Stages: Where Your Receding Hairline Fits

Doctors use the Norwood-Hamilton scale, which runs from stage I to stage VII, to classify hereditary hair loss in men. The early stages are particularly relevant to a receding hairline, as a hair transplant can be especially effective at this point and the donor area still has plenty of hair.

The overview below can help you estimate your stage. Our innovative 6-step Pre-Test System provides a precise assessment, with each measurement informing a specific treatment decision. We use those results to develop your individual treatment plan.

Norwood scale showing head icons for stages 1 to 7, with stages 2 and 3 highlighted as the typical range for a receding hairline
Norwood stageWhat you see in the mirrorWhat it means for a receding hairline
Norwood 1
Full, youthful hairline with no recession
This stage alone does not indicate any recession to treat
Typical range for a receding hairline
Norwood 2
Slight, symmetrical recession at the temples, with an M shape beginning to form
Early but established recession at the temples, readily treatable
Norwood 3
Pronounced, deep recession with a distinct M shape; the first clinically diagnosable stage
A common stage for hairline-restoration enquiries
Norwood 3 vertex / 4
Receding hairline plus early thinning at the crown
May require planning for both the frontal area and crown
Norwood 5 to 7
Extensive hair loss, leaving a horseshoe-shaped rim of hair
Advanced loss; donor preservation and coverage priorities become more important

For Norwood 2 to 3 hair loss, a transplant focused mainly on the receding areas at the temples is often considered. Medication may help preserve or improve miniaturised native hair, but it has limited scope to restore hair in temple areas that have been completely bald for a long time. A transplant can move follicles into these areas if your donor supply and long-term treatment plan allow.

Medication, PRP or Surgery: What Each Option Can Do

Two test tubes of PRP plasma and an injection at a woman's hair roots

Many products promise to reverse a receding hairline. In practice, non-surgical treatments can stabilise hair loss and thicken the hair you still have, but meaningful regrowth in areas that have been bald for a long time is unlikely. The overview below explains what each option can do and where its limitations lie.

Minoxidil (topical)

What it does: May prolong the growth phase and improve density in some miniaturised hairs. Limitations in bald temple areas: Ongoing treatment is generally needed to maintain the benefits; areas that have been bald for a long time show a limited response.

Finasteride (oral, men only)

What it does: Lowers DHT levels and stops or slows hair loss in around 90% of men. Its limitations for bald areas at the temples: Primarily helps preserve existing hair rather than restore hair in bald areas at the temples.

PRP (platelet-rich plasma)

What it does: May support hair density in some patients. Limitations in bald temple areas: An adjunct rather than a substitute for transplantation in a completely bald area.

Hair Transplant

What it does: Redistributes donor follicles that are generally more resistant to androgen-related miniaturisation. Role in bald temple areas: Can provide long-term hair coverage in selected bald areas using the patient's own donor follicles.

Medical treatments may preserve or thicken native hair that is susceptible to hair loss, while a transplant moves donor follicles into selected areas. The two approaches can complement each other.

Discuss with your doctor whether medication alongside a transplant is suitable for you. This helps maintain a balanced overall appearance in the long term and reduces the risk of new gaps developing around the transplanted hair.

Grafts

How Many Grafts You Need for a Receding Hairline

Close-up of a hair follicle graft held with forceps in theatre

As a general guide, a receding hairline may require 2,000 to 3,500 grafts. Milder Norwood 2 recession may require roughly 2,000 to 2,500 grafts, while more pronounced Norwood 3 recession may require about 2,500 to 3,500. These are broad estimates rather than fixed requirements, and many suitable cases can be treated in one session.

A graft is a naturally occurring follicular unit containing one to four hairs, with an average of about 2.2. The number you need depends on how far your hairline has receded. The table below provides a realistic estimate based on your Norwood stage.

2,000–3,500Grafts for a receding hairline

Norwood stageSeverityGrafts
Norwood 2

2,000 to 2,500
Norwood 3

2,500–3,500
For context

For most cases of temple recession, you will need 2,000 to 3,500 grafts, and a single session is sufficient. To answer a common question: 2,700 grafts is a typical mid-range figure for the temples and hairline. Much higher figures, such as 5,000 grafts, apply to large bald areas rather than a receding hairline alone.

second session

A second session may be considered if your hair loss has progressed, you want greater density or your treatment is being carried out in stages. A second treatment, including one to add density, takes place no earlier than six months after the first.

Donor Area

Many suitable receding-hairline cases can be treated in one session, and more grafts are not automatically better. Excessive extraction can create visible thinning in the donor area (Okochi et al. 2020). A well-planned procedure takes only as many grafts as the area being treated needs, preserving donor hair for future treatment (Unger et al. 2013).

Most clinics charge per graft, so you may not know the final price until the graft count is confirmed. Elithair uses all-inclusive pricing tiers based on graft count, so you know exactly what to expect before you travel.

For the typical receding-hairline range of 2,000 to 3,500 grafts, current all-inclusive package prices run from £2,849 to £3,149.

For organisational reasons, your flight is not included; depending on your flight route, it typically costs around £90 to £220. Zero Shave FUE and Manual FUE are optional extraction methods available at an extra charge.

Hair transplantation is not available on the NHS because it is cosmetic surgery.

Hairline

Hairline Design at the Forehead and Temples

The hairline is the most demanding part of the procedure. An incorrect angle or a misplaced graft is immediately noticeable. Careful design makes the difference between an obvious transplant and a result that looks completely natural.

A natural hairline is never ruler-straight (Shapiro et al. 2013). It has gentle curves, a subtle micro-zigzag pattern and a soft transition zone around half a centimetre to one centimetre deep.

We fill this transition zone exclusively with single-hair grafts containing the finest hairs from the donor area, creating a soft transition rather than a harsh, doll-like edge. Behind them, we place 2-hair grafts for fullness, followed by stronger 3- and 4-hair grafts further back. The angle also follows nature: grafts in the front row are placed almost flat against the scalp at around 10 to 15 degrees, with the angle gradually increasing behind them.

The temples are a common source of mistakes when designing a male hairline. Slight recession at the temples is a natural feature of the male face. Filling them in too straight or too low can create a hairline that looks unnatural, especially as you age. At Elithair, the doctor designs the entire hairline personally to suit your age and face shape, helping the result remain balanced for decades.

Technique

How FUE, DHI and Post-Procedure Care Fit Together

At Elithair, every treatment follows a set sequence: the six-step Pre-Test System on arrival, FUE extraction of grafts from the back of your head, DHI implantation followed by aftercare with the patented NEO FUE® Serum.

Good to know: FUE and DHI describe different stages of the same treatment: FUE refers to graft extraction, while DHI refers to implantation using the CHOI Implanter Pen. The overview below explains the role of each stage.

The patented NEO FUE® Serum is included in aftercare for UK patients. You receive it at the clinic together with a dermaroller set and apply it to your scalp on day 14 and day 28 after the procedure, following the clinic's instructions. The serum supports healing.

  2. six-step Pre-Test System

In Istanbul, your treatment starts with the six-step Pre-Test System. The six checks comprise an electrocardiogram (ECG), a general health blood test, a blood test relating to hair loss, hair analysis with the TrichoLAB scanner, a scalp ultrasound (skin thickness) and a Doppler ultrasound (blood circulation). Based on the results, our doctors draw up your individual treatment plan.

    approx. 98%

Skin thickness and blood circulation together determine the insertion depth, which underpins a graft survival rate of approximately 98%.

  3. DHI (CHOI Implanter Pen)

DHI is particularly effective at the hairline. The CHOI Implanter Pen opens a channel and inserts the graft in a single step. The implantation angle is controlled directly during placement, including the 10 to 15 degrees that make the front row look natural. Individual hairs can be placed precisely, follicle by follicle, and higher densities can be achieved in the transition zone than with older methods.

Process

Hair Transplant for a Receding Hairline: What to Expect at Elithair

  1. Free Hair Analysis

Send us photos of your receding hairline so our doctors can estimate the number of grafts you need and the cost. Together, we choose a date that works for you, with no obligation.

  2. Pre-Test System on arrival day

We pick you up at the airport and take you straight to the clinic for the six-step Pre-Test System: an electrocardiogram (ECG), a general health blood test, a blood test relating to hair loss, hair analysis with the TrichoLAB scanner, a scalp ultrasound (skin thickness) and a Doppler ultrasound (blood circulation). Skin thickness and blood circulation together determine the insertion depth. Together with a clinical examination, the results form the basis of your individual treatment plan.

Afterwards, we take you to your hotel. If you land late in the evening, we still bring you to the clinic for your blood tests, and the remaining Pre-Test steps follow on day 2.

  3. The procedure

A seated man's marked hairline being measured with a metal measuring instrument.

On the day of treatment, your hairline is first planned to the millimetre using your measurements and drawn with your input. The procedure then takes place on an outpatient basis, in one day, without general anaesthesia. It begins with about 15 minutes of twilight sedation using the Sleep-Deep method. You sleep through the administration of the local anaesthetic and experience 0% pain during the procedure. Afterwards, you are awake but pain-free.

The treatment takes around 5 to 8 hours, depending on the number of grafts. Under our 5-Doctor Standard, our doctors oversee and take responsibility for your treatment at five key points.

  4. Follow-up and departure

Follow-up check at Elithair: a doctor examines a patient's scalp after the hair transplant

You return to the clinic the next day for aftercare and a medical check-up, and fly home on day 4.

  5. Healing and Aftercare

Our medical team monitors your recovery through the Elithair App for 12 months. You upload progress photos for the team to review, and push notifications remind you when you can resume each activity.

The timeline below shows how healing progresses.

Common mistakes

Risks and Common Mistakes to Be Aware Of

Doctor in a white coat pointing to a planned hairline on an image of a head displayed on a large screen

Hair transplantation is generally considered safe when appropriately planned and performed, but it is still surgery and complications or unsatisfactory cosmetic outcomes can occur.

A hairline that is too straight or too low

This can look unnatural as you age. We address this with an age-appropriate design drawn specifically for you by the doctor.

Devil's horns

"Devil's horns": if you have surgery too early, while hair loss is still active, you can eventually be left with only the transplanted hair at the temples as the hair behind it continues to thin.

Shock Loss

In the first few weeks after the procedure, transplanted hair shafts may temporarily fall out while the follicles remain in place, although not every patient experiences this. This is a normal stage of recovery, though many people find it unsettling.

Removing too many grafts

Removing too many grafts can damage the donor area at the back of your head. A well-planned procedure takes only as many grafts as the area being treated needs.

All these issues can be avoided through careful planning, honest advice and experienced doctors.

Hair Loss in Women

A Receding Hairline in Women

Female patient wearing a face mask with a hairline marked on her forehead in front of an Elithair logo wall

Women with frontal recession should have the cause assessed carefully before transplantation is considered (Lam 2020).

Women can develop a receding hairline too, though it is less common than in men and requires a different approach to treatment planning. A woman's hairline has a different shape, lower in the centre and higher at the sides. Hereditary hair loss more often appears as diffuse thinning along the parting than as the typical recession at the temples. Whether a transplant is suitable therefore depends heavily on the exact cause.

A receding hairline in women can also indicate a scarring form of hair loss, such as frontal fibrosing alopecia. In these cases, a hairline transplant is not appropriate while the condition is active, because the same process can destroy the transplanted follicles.

That is why women always need a thorough dermatological assessment first (Lam 2020). Our medical specialists provide this as part of your individual hair analysis.

FAQ

Frequently Asked Questions About Hair Transplants for a Receding Hairline

Elithair offers all-inclusive packages priced by graft range. For a receding hairline, the typical range is 2,000 to 3,500 grafts, with current package prices ranging from £2,849 to £3,149. The package includes the procedure, three nights at a 4-star hotel near the clinic as part of the four-day itinerary, local transfers, the six-step Pre-Test System, one PRP treatment, medication and care products, a clinical follow-up before departure and 12 months of aftercare through the Elithair App. The included PRP treatment takes place at the clinic on the day of your procedure. Your flight is booked separately and typically costs around £90 to £220, depending on your flight route. Zero Shave FUE and Manual FUE are optional extraction methods available at an extra charge.

It depends on how far your hairline has receded. Mild recession (Norwood 2) usually requires 2,000 to 2,500 grafts; more pronounced recession (Norwood 3) requires roughly 2,500 to 3,500. Around 2,700 grafts is a typical mid-range figure for the temples and hairline, and a single session is sufficient. The doctor estimates the number during your free hair analysis and confirms it during the Pre-Test at the clinic, so only as many grafts as the area needs are extracted and your donor area is preserved.

Usually not. Because only a limited area is treated for a receding hairline, a full shave is often unnecessary. Usually, only a narrow section of the donor area is trimmed short, which the surrounding longer hair then conceals. At the front, we place grafts directly between your existing hairs. Our optional Zero Shave FUE extraction method avoids shaving entirely and is available for an additional £450 when fewer than 3,500 grafts are needed. We determine whether it is suitable during your free hair analysis.

No. Elithair uses the Sleep-Deep method, so you experience 0% pain during the procedure without general anaesthesia and its associated risks. You sleep through the most uncomfortable step: administration of the local anaesthetic. Your scalp may feel slightly tight for a few days afterwards, but this can be managed well with the medication we provide.

The procedure takes place on one treatment day during your four-day, three-night stay in Istanbul. The treatment itself takes around 5 to 8 hours, depending on the number of grafts. Temporary shedding can occur in the first few weeks after the procedure, although not every patient experiences it. New growth begins by month four at the latest, becomes more noticeable from around month six, and the full result at the front of the scalp is usually visible after 10 to 12 months.

Yes. The hairline is the most demanding part of the procedure, and at Elithair it is designed entirely by the doctor. We use only fine single-hair grafts in the transition zone, placing them at a shallow angle of 10 to 15 degrees. The hairline is deliberately designed to suit your age and retain natural recession at the temples, helping the result remain balanced for decades.

Transplanted follicles generally retain the donor area's greater resistance to androgens and can last long term. Elithair provides a 20-year written guarantee on transplanted hair, subject to the guarantee terms and aftercare requirements. Native hair around the transplant can still thin, so long-term planning may include discussion of medical treatment where appropriate.

Your hair transplant takes place at Elithair's clinic in Istanbul. Under the 5-Doctor Standard, our doctors oversee and take responsibility for your treatment at five key points. The six-step Pre-Test System forms the basis of your individual treatment plan.

Hair transplantation is not available on the NHS because it is cosmetic surgery. Private insurance cover depends on the policy; elective cosmetic treatment is commonly excluded.
