Afro Hair Transplant: Natural Results for Afro-Textured Hair

An Afro hair transplant should give you a result that blends seamlessly and naturally with your features. From £2,849, we transplant your own hair follicles using an extraction technique that follows the curved path of your hair roots.

Because Afro hair roots curve beneath the skin, the procedure requires specialist expertise. It's also essential to establish the cause of your hair loss first, as surgery isn't suitable for every type of hair loss.

With our free, no-obligation hair analysis, we assess whether a transplant may be suitable for you and whether your donor area can support the treatment plan.

Afro hair

Afro Hair Transplant Before and After

With coily hair, a natural-looking result depends less on the number of grafts than on how well the transplanted hair blends with your existing hair. The hairline is slightly irregular, never a hard, straight edge. The front row is made up of single, fine hairs, with density building behind them. Your temples keep their original shape instead of being rounded off.

The transplanted hair matches the tight curl pattern and growth direction of the surrounding hair, including at the crown and along the parting. There is no gap between the treated areas and your existing hair, and the donor area looks even, without thinner patches.

Visual effect

Thanks to the curl pattern, every graft provides particularly high visible coverage.

Pain & scarring

Local anaesthesia under Sleep-Deep sedation, 0% pain. No scarring. A tendency to form keloids is checked in your medical history beforehand.

Process & investment

Three days of full-service care in Istanbul. All-inclusive price: £2,849 to £3,449.

Growth & guarantee

First growth by month 4 at the latest. Final result at the front after 10 to 12 months (crown: 15 to 18 months). Includes a 20-year written guarantee on the transplanted hair.

How an Afro Hair Transplant Works

An Afro hair transplant moves your own tightly curled or coily hair from a suitable donor area to areas affected by hair loss. Follicles are selected from parts of the scalp that are generally less affected by DHT (dihydrotestosterone) and then transplanted into the recipient area.

Once transplanted, they retain their natural resistance to hereditary hair loss.

Our treatment approach combines two established methods for optimal hair growth. Using FUE, follicles are extracted individually. No scars are left.

The grafts are then implanted using DHI with the CHOI Implanter Pen. Channel creation and placement take place in one step, allowing the medical team to control the planned angle, direction and depth of each graft.

FUE and DHI aren't alternatives to choose between. They're two phases of one well-practised process, and both are included as standard in our all-inclusive price.

Afro-textured hair

The Anatomy of Afro Hair: The Hidden Path of the Root

The visible curl shapes how your hair looks, but what really matters lies beneath the surface. Afro-textured hair has an oval to elliptical cross-section, while straight hair is closer to round.

The characteristic curve begins deep within the skin. A scientific review by Cloete, Khumalo & Ngoepe (2019) shows that the bend starts at the very base of the root. Depending on your genetics, follicles can follow anything from a gentle arc to a tight spiral through the tissue.

Further micro-CT studies by the same research group (2025) classify curly hair fibres by curvature type. One key finding is that ancestry does not reliably predict curl type; the structure of the individual hair fibre is what matters. The same principle guides our approach to treatment: we focus on the actual path of your hair roots.

In practice, this leads to one essential rule: with Afro-textured hair, the angle at which a hair exits the skin tells you nothing reliable about the direction of the root below. To extract the follicles intact, the procedure must take precise account of this hidden curve.

visible coverage

Visible Coverage and Individual Graft Planning

Afro-textured hair has a special advantage when it grows: its three-dimensional curl pattern makes individual hairs overlap naturally, creating visible fullness.

As the International Society of Hair Restoration Surgery (ISHRS) confirms, coily hair often needs fewer follicles to provide effective scalp coverage.

Achieving a full look isn't about restoring your original number of hairs. It's about closing areas where the scalp shows through. Marketing claims that quote exact percentages aren't credible here. The evidence supports the principle of greater volume, but not a single figure that applies to everyone. To get a rough figure for your case, use our graft calculator.

At the same time, planning requires a close look at the back of your head: research on hair density (Loussouarn, 2001) puts the average follicle density of Afro-textured hair at about 190 hairs per square centimetre. Greater visible coverage therefore doesn't automatically mean that fewer grafts are needed. Careful planning based on your donor area remains essential.

With the TrichoLAB analysis in our Pre-Test System, we measure your hair root density in four regions of your scalp. This shows how many grafts your donor area can provide and how many your treatment plan needs.

Who Is a Candidate for an Afro Hair Transplant?

Every successful hair transplant requires two things: the cause of your hair loss has to be clearly identified, and your donor area has to remain stable over time. This diagnosis is particularly important for people with Afro-textured hair because, in addition to hereditary thinning, two other forms of hair loss are more common and require completely different approaches to treatment.

Hereditary hair loss

Androgenetic alopecia

Typical signs

Receding hairline, receding temples or thinning on top with a dense band of hair around the sides and back.

What it means for treatment

The classic case: because the follicles in the donor area are insensitive to the hormone DHT, the procedure can be planned precisely and with lasting results.

Traction alopecia

caused by pulling

Typical signs

Thinning at the hairline, temples or edges, exactly where tight hairstyles pull.

What it means for treatment

As long as the tissue isn't scarred, relieving the tension and targeted care come first. A transplant only makes sense once the condition is completely stable.

CCCA & scarring forms

Typical signs

Thinning at the centre of the scalp, sometimes with shiny skin, itching or mild burning.

What it means for treatment

No transplant during the active, inflammatory stage. It may only be considered once the condition has been shown to be stable and after a clear assessment of your suitability.

Unclear or diffuse causes

Typical signs

Sudden, widespread hair loss, round bald patches or unusual accompanying symptoms.

What it means for treatment

A thorough diagnosis comes first. We don't perform a transplant until the cause is clear.

This overview is for guidance only and does not replace a medical assessment. If the cause of hair loss is unclear, further dermatological evaluation, including dermoscopy or a biopsy where clinically indicated, may be needed before a transplant is considered.

less invasive alternatives

If a Transplant Isn't an Option: Your Alternatives

Two test tubes of PRP and a woman receiving an injection at the hair roots

If a transplant isn't the right option for you, there are less invasive alternatives: professional scalp micropigmentation uses fine pigments to make thinning areas appear denser, without any surgery.

PRP may be considered as a non-surgical option for some types of hair loss, although outcomes vary and it is not a substitute for diagnosing the underlying cause. The PRP session included with an Elithair hair transplant takes place at the clinic on the day of treatment.

Constant Pulling

Traction Alopecia: When Constant Pulling Weakens Your Hair

Traction alopecia develops when mechanical tension pulls on the hair roots over a long period. Common triggers include tight braids, cornrows, weaves, extensions and tightly pulled-back styles.

The damage usually develops gradually in characteristic areas: along the hairline, at the temples and around the edges. Dermatological studies show how common it is: signs of traction alopecia at the hairline were found in 17.1% of the schoolgirls and 31.7% of the adult women examined.

The combination with chemical relaxers is especially harmful, because weakened hair breaks even faster under tension. Early warning signs on the scalp include a tight feeling after styling, redness or small bumps along the edges. Early traction alopecia can be reversible when the source of tension is removed, although recovery depends on the extent and duration of follicle damage.

In the early stage: the follicles may still recover if the source of traction is removed. The most important step is to relieve the tension consistently by choosing looser styles, alongside dermatological care. At this stage, the hair often grows back on its own.

In the advanced stage: if a follicle has been damaged over many years and replaced by scar tissue, no new hair will grow from it. A transplant only makes sense once the pulling has stopped for good and the tissue has settled down. Otherwise, the newly implanted follicles would fall victim to the same tension.

CCCA: Scarring Hair Loss on Top of the Head

A transplant therefore always requires a dermatological evaluation first and an individual decision by a doctor (Lam 2020).

According to StatPearls, central centrifugal cicatricial alopecia (CCCA) affects about 2 to 7 per cent of women with Afro-textured hair. It spreads outwards in a circle from the crown and is often accompanied by itching or burning. The cause lies primarily in genetic factors, such as a variant of the PADI3 gene (2019), rather than styling habits.

Strict medical criteria apply to a hair transplant:

No procedure while inflammation is present: according to DermNet, the condition should be well controlled for at least one year before the procedure.

Higher risk: grafts take hold less easily in scarred tissue. Case reports (Callender et al., 2014) and meta-analyses (Ekelem et al., 2019) show good results in inactive CCCA, but also a higher failure rate.

A transplant therefore always requires a dermatological evaluation first and an individual decision by a doctor (Lam 2020). Learn more about treatment planning on our page about hair transplants for women.

darker skin

Keloids and Pigmentation: What to Know If You Have Darker Skin

Keloid formation after FUE has been reported but appears to be uncommon.

Darker skin is more prone to keloid formation (about 3.5% in a study from Cameroon), but your individual risk depends on how your wounds have healed in the past.

Systematic reviews of FUE complications (2026) confirm that keloids are extremely rare after FUE because, unlike the strip method, the tiny extraction sites heal without tension.

We still carefully check for any tendency towards keloids during your initial consultation.

Protecting Against Pigment Changes After the Procedure

On darker skin, irritation can temporarily leave lighter or darker marks in the donor area, as well as deep redness.

To prevent post-inflammatory pigment changes, UV protection is essential: for the first two weeks, always wear the protective hat we provide when you go out.

Root Curvature

The Challenge of Root Curvature in FUE

Person in a white coat measuring the forehead of a woman with dark hair

In FUE, the grafts are removed one by one from the donor area with a fine hollow punch. Because Afro-textured hair roots curve beneath the skin, a rigid, shallow insertion angle can easily cut through the follicle (transection).

Research shows that FUE can work extremely well on coily hair. A study by Dr Sanusi Umar (2016) demonstrates that adapted instruments can reduce the transection rate from over 60% to under 5%. Because robotic systems such as ARTAS cannot reliably handle curly hair structures, the practitioner's skill is critical.

As described in Hair Transplant Forum International (Reyes, 2021), each curvature type requires individual adjustment of punch diameter, angle and depth.

Manual precision

Low Speed and Manual Precision at Elithair

Close-up of FUE extraction at the back of the head with a punch tool and gloved hands

Because curved follicles can be more difficult to extract, careful control of punch angle, depth and movement is particularly important. At Elithair, the medical specialists carrying out this step have at least seven years of experience.

We also reduce the micromotor speed to protect the surrounding tissue.

For even greater control, there's manual extraction (Manual FUE), in which each graft is removed by hand. It's available as an option for an additional fee.

DHI technique

Precise Implantation with the DHI Technique

FUE is used to extract the grafts, but the subsequent implantation determines how natural and dense your final result looks.

Our standard is the modern DHI technique with the CHOI Implanter Pen (Choi et al. 1992). The key advantage: opening the channel and placing the follicle happen in one smooth step.

DHI allows planned control of depth, angle and direction during implantation, which is important when matching the natural growth pattern of Afro-textured hair. Because the follicle is inserted directly into the scalp while remaining protected, there's no need to open a separate channel or place the graft with tweezers.

The aim is for the transplanted hair to follow the planned growth direction and blend with the surrounding hair, with implantation depth based on the individual treatment plan.

Pre-Test System

Two Steps to Your Personalised Treatment Plan

The process begins with a free online analysis: based on your photos, our doctors assess whether you're suitable for a hair transplant and estimate how many grafts you may need and how much the procedure will cost.

In Istanbul, you go through our 6-step Pre-Test System at the clinic on the day you arrive: an electrocardiogram (ECG), a general health blood test, a hair loss blood test, a hair analysis with the TrichoLAB scanner, a scalp ultrasound (skin thickness) and a Doppler ultrasound (blood circulation). Based on the results, our doctors create your individual treatment plan.

Rather than estimating insertion depth, our doctors calculate it based on your skin thickness and blood flow, then record it in your treatment plan. When grafts are placed too shallowly, the survival rate is often only about 60%. The calculated insertion depth and NEO FUE protocol together contribute to a survival rate of approximately 98%.

In-House Innovations

Two Innovations for Your Treatment Success

To achieve the best possible results while making your time at the clinic as comfortable as possible, we use two in-house innovations. One supports healing in the donor and recipient areas, while the other keeps pain during treatment at 0%.

Two ampoules of Dr Balwi's NEO FUE® Serum with the dermaroller set
NEO FUE® Serum

Our patented NEO FUE® Serum was developed by Dr Abdulaziz Balwi. You receive the set, containing a dermaroller and two ampoules of serum, at our clinic in Istanbul and apply it to your donor and recipient areas exactly twice: on day 14 and day 28 after your procedure. It supports healing and, together with the insertion depth determined through your Pre-Test System, contributes to the survival rate of approximately 98%.

Patient relaxing under Sleep-Deep sedation, wearing a surgical cap and a nasal cannula
Sleep-Deep®

Sleep-Deep® completely eliminates the discomfort of the most unpleasant part of the procedure: the local anaesthetic injections. With Sleep-Deep sedation, you drift into a light, sleep-like state for about 15 minutes under the supervision of a specialist anaesthetist. You sleep through the local anaesthetic and feel 0% pain during your procedure.

Recovery and Growth

Recovery and Growth: Your Healing Timeline at a Glance

Healing after an Afro hair transplant follows the same biological stages as with any other hair type. To help you feel prepared during the first few weeks, we have set out each stage below so that you know what to expect and what matters from day one.

Procedure day & day 1
Rest lying down and sleep on your back with a neck pillow. A reddish fluid may seep from the donor area; it's mostly harmless saline solution.
Days 2 to 8
First gentle hair wash on the second day after the procedure, then daily, strictly following instructions. Swelling usually goes down completely within a week.
Day 9
Take a one-day break from washing to allow the scabs to form properly.
Days 10 & 11
Targeted scab removal in the recipient area with the special lotion (never with your fingernails). By the end of day 11, all scabs have been gently removed.
From day 12
You can carefully wash your hair with regular shampoo again.
Up to month 3
In the first weeks after the procedure, the transplanted hair shafts may fall out temporarily (shock loss), while the follicles stay firmly anchored in the scalp. Shock loss can occur, but not every patient experiences it (Okochi et al. 2024). Visible coverage is around 10% at this stage.
By month 4 at the latest
The actual growth phase begins, with an average of 10% new growth per month.
Month 6
The first result is visible, and the donor area has fully healed.
Months 10 to 12
The final result at the front and along the hairline is in place.
Months 15 to 18
Final result at the crown (top of the head).

Styling and Hair Care After Your Transplant

Aftercare for curly and coily hair is a topic many other clinic websites don't address. For the first 11 days, wound healing takes priority over your usual hair care routine, so it's important to follow the medical washing schedule strictly.

From day 12, you return to your usual routine. For coily hair, dermatologists often recommend a gentle weekly wash and silk pillowcases or a silk scarf at night.

The following timelines apply to cutting, styling and headwear:

Timelines for cutting, styling and headwear after your transplant
WhatWhen
Headwear For the first 14 days, only the hat from the clinic: no durag and no tight scarf. Wear it in direct sun, in dust and at temperatures of 24°C and above; at 40°C and above, for one month.
Scissors Donor area after 1 week; recipient area after 2 weeks.
Clippers Donor area after 2 weeks; recipient area after 1 month.
Razor & clean shave Donor area after 2 weeks; recipient area after 3 months.
Blow-drying Month 1: not at all; month 2: cool air only; from month 3: no restrictions.
Straightening After 3 months.
Styling the donor and recipient areas Recipient area after 6 months; donor area after 1 month; existing hair outside the treated zone after 2 weeks.
Wig After 6 months

Tip for short cuts: you can have your donor area cut with clippers after 2 weeks, but for the first 6 months, don't go shorter than 1 cm.

Protecting Against Tension (Braids, Cornrows, Extensions)

Newly placed roots are extremely sensitive to tension. Tight braids, weaves, extensions and chemical relaxers are off-limits for the first few months to avoid traction alopecia. In future, always braid loosely and give your scalp regular breaks.

Finding the Cause Early

Why Finding the Cause Early Matters with Afro Hair

Deciding whether to have a hair transplant takes careful consideration. But timing matters with Afro hair, too: the sooner the cause is identified, the more of your own hair roots can be saved, especially with traction-related hair loss. Scarred tissue can't be brought back later.

Your free hair analysis gives you clarity straight away. Since the final result at the front takes around 10 to 12 months, every month saved brings your desired result closer.

Afro-textured hair can be transplanted, and the process is the same as for any other hair type: FUE extraction from the donor area and DHI implantation into the recipient area (Umar et al. 2023). The difference lies in how the procedure is performed: because the hair root curves beneath the skin, the extraction must follow its path (Rogers et al. 2014). The prerequisites are that the cause of your hair loss has been identified and your donor area is stable.

Extraction takes more experience, because the visible exit angle doesn't show how the root runs beneath the skin. With an adapted approach, however, the rate of cut hair roots stays within the normal range. At Elithair, grafts are extracted only by medical specialists with at least seven years of experience in this step, at 100 to 200 revolutions per minute.

Coily hair covers more area per graft than straight hair, so the same number of grafts looks fuller. There is no single percentage that applies to everyone, and stronger visual coverage does not mean fewer grafts should automatically be planned. The number of grafts you need depends on the size of the affected area and the condition of your donor area. We assess this during your free, no-obligation hair analysis.

Yes. We transplant your own hair root with all its characteristics. Its curl, thickness and colour remain the same because these characteristics are determined by the root itself, not by its location.

It depends on the stage. As long as the hair roots are irritated but not scarred, an operation isn't the right path. What matters then is stopping the tension for good and getting dermatological treatment, because at this stage the hair often comes back. If the area is scarred and hasn't changed for a long time, a transplant may become an option. A doctor makes this assessment after an examination.

Not during the active, inflammatory stage, because the disease process would also affect the transplanted hair roots. The medical reference DermNet names at least one year of good disease control as a prerequisite, and the medical literature notes that hair roots take hold less well in scarred tissue. That's why the first step is always a dermatological evaluation; the decision is then made case by case.

Not automatically. What matters is your personal and family history, that is, whether previous scars, ear piercings or surgical wounds healed with raised scars. Keloids after FUE have been reported but appear to be uncommon. Your personal and family history of keloid scarring is therefore an important part of the suitability assessment. We review your history before confirming your treatment, and if in doubt, we advise against it.

Relaxing doesn't rule out treatment, but it's something we look at in your hair analysis, because the combination of relaxers and tension is a proven risk factor for traction alopecia, so the cause of your hair loss has to be clarified. You can straighten your hair again three months after the procedure at the earliest; whether that also applies to chemical relaxers is something to discuss with your treatment team beforehand.

Usually, yes. If you already wear your Afro hair short, the change may be minimal. A procedure without shaving using Zero Shave FUE is available for suitable cases requiring fewer than 3,500 grafts, for an additional £450. Medium-length or long hair is not a requirement. During your hair analysis, we assess whether this method is suitable for your hair structure.

If you need to cover your head in the first 14 days, wear only the hat from the clinic. Our aftercare plan doesn't give separate timings for braids, cornrows and extensions, but the guidance is clear: avoid any tension on the treated area for the first few months, and wait six months before styling the recipient area again. Talk to your treatment team about when you can wear which style.

Your donor area can be cut with scissors again after one week and with clippers after two weeks. In the recipient area, you wait two weeks for scissors and one month for clippers. A clean shave in the recipient area is possible after three months. For the first six months, don't have your donor area cut shorter than 1 cm.
