At Elithair, medical responsibilities are clearly assigned: our experienced physicians oversee and take responsibility for the five most important steps of your treatment, without exception.

That is the 5-Doctor Standard at Elithair: defined medical responsibilities, consistent clinical oversight, and individualized treatment planning.

Medical Team

Why Choosing the Right Medical Team Matters

Hair transplantation is not a separate medical specialty in either the US or Turkey. How a procedure is organized, and who handles which part of it, varies enormously from clinic to clinic.

Court rulings and media reports repeatedly highlight the need to clearly distinguish tasks that only a doctor should perform from those assistants can handle. Your safety and the success of your treatment depend on clear rules.

Before you book with any clinic, you should know exactly how your procedure will be performed and who provides medical oversight at each step. Our 5-Doctor Standard gives you that clarity, so you can go into your treatment with confidence.

Medical Assessment

Phase 1: Medical Assessment Before You Travel

Our doctors review your case in detail before you plan your trip. That review starts with the photos you submit for your free, no-obligation hair analysis.

They look at three things: whether treatment makes medical sense right now, whether your donor area is dense enough, and whether the result you want can be achieved with the number of grafts available.

We only promise results we can realistically deliver, so we decline about 40 percent of inquiries. When we confirm your appointment, we have already completed a thorough medical assessment.

Pre-Test System

Phase 2: The Pre-Test System Behind Your Personalized Surgical Plan

Right after you arrive in Istanbul, you undergo a six-step examination at our clinic. Our innovative Pre-Test System provides the precise medical data we use to create your personalized treatment plan.

Pre-Test System examinations: what is measured and what it means for your treatment
Examination What is measured What it means for your treatment
Electrocardiogram (ECG) The electrical activity of your heart Your treatment plan takes your overall health into account, so you're well prepared for the procedure
Blood test for general health Infectious diseases such as hepatitis B and C, HIV and syphilis We ensure safe treatment conditions for you, other patients and the medical team
Blood test related to hair loss Possible causes of your hair loss The results are factored into your personalized treatment plan
Hair analysis with the TrichoLAB scanner Hair shaft thickness and multi-graft ratios in four regions We determine how many single-, double- and triple-hair grafts are available so we can plan a natural hairline with even density
Scalp ultrasound The thickness of your scalp Combined with blood-flow measurements, informs graft implantation depth
Doppler ultrasound Blood flow in the main arteries of your scalp Together with skin thickness, this determines the ideal insertion depth for your grafts
Personalized Surgical Plan

Why This Process Is Indispensable

The last three examinations make this process indispensable. They show how many grafts of each type are available and how deeply each graft should be placed.

Rather than using a standard treatment plan, we create a personalized surgical plan based on your measurements.

Hairline Design

Your Doctor Designs Your Hairline

Your hairline shapes the overall appearance of your face. A hairline that is too low or perfectly straight looks artificial and becomes less flattering as you age.

At Elithair, a doctor designs your hairline, determining its height, the density in each area and the fine details that help it complement your facial features. Single-hair grafts are placed precisely along the front, with multi-hair grafts behind them to create the volume you need.

We design your hairline so it will still suit your features naturally twenty years from now. If a request conflicts with that goal, our doctors will tell you openly and advise you on an alternative that will look good for the long term.

Anesthesia Led by an Anesthesiologist

Pain-free anesthesia helps you stay relaxed on the day of treatment and requires the highest level of medical care.

At Elithair, an anesthesiologist always manages Sleep-Deep sedation, without exception. It puts you into a calm, sleep-like state for about 15 minutes while you receive the local anesthetic. You sleep through the injections and feel no pain.

The anesthesiologist continuously monitors your heart and circulation throughout the procedure. At your request, or if medically necessary, we can offer the needle-free Comfort-In anesthesia method instead.

Physician-Led Surgery

Phase 5: Physician-Led Surgery and Final Check

At Elithair, both extraction and implantation follow a strict in-house protocol under the continuous supervision of the lead doctor.

This final examination helps protect the healing process and ensures you go home with all the essential information and aftercare instructions.

The doctor checks and signs off on every stage individually. During extraction, for example, they personally confirm that exactly the required number of grafts has been harvested and that the grafts are of the highest quality before implantation begins.

  2. Grafts being placed into the marked scalp with a DHI pen
    Implantation

Before the procedure ends, the doctor checks that the transplanted follicles have the right direction and density.

On the day you leave the clinic, the doctor performs a final examination of your donor and recipient areas and signs off on your care.

Consistent Quality Standards

High standards of medical care depend on reliable processes. Because every patient undergoes the same diagnostic steps and checkpoints, quality can be monitored continuously at every phase of the procedure.

We tailor every detail of your treatment to you in advance, precisely calculating graft angles, the extraction pattern, tissue depth and the shape of your hairline.

Subtle anatomical differences mean that no two treatment plans are alike, even when patients need exactly the same number of grafts. This helps ensure your result continues to suit you over time.

Transparency

What the 5-Doctor Standard Is Not

Lasting trust starts with transparency. That's why we're honest and clear with you, including about what the 5-Doctor Standard does not mean:

  • It doesn't mean five doctors are in the treatment room at the same time.

It means your treatment includes five essential medical checkpoints for which a physician takes full responsibility.

  • It doesn't mean a doctor places each graft individually.

It means a doctor sets the exact specifications for the depth, angle and density at which the grafts are placed.

  • It doesn't mean Dr. Balwi performs every treatment personally.

As medical director, he is responsible for the treatment procedures we follow and their ongoing development.

  • It doesn't mean every inquiry leads to treatment.

We do not offer appointments to around 40 percent of prospective patients because we only proceed when the conditions are right for treatment.

  • It doesn't mean standardized processes leave no room for your preferences.

Consistent steps provide a framework for safety, while your treatment plan remains fully tailored to you.

  • It doesn't mean doctors are involved only on paper.

Each of the five physician checkpoints requires an active assessment and personal sign-off, with uninterrupted supervision throughout treatment.

Checklist

Your Checklist: Questions for Your First Consultation

To help you choose the right clinic, we've put the key recommendations from independent medical associations into a list of questions. A transparent provider will give you clear, reliable answers to each one:

Doctor in a white coat advising a female patient together with the medical team
  1. Medical responsibility:

Who designs and draws my hairline? Does a doctor do both?

  2. Team experience:

Who extracts the grafts and opens the recipient channels, and how much experience do they have with these steps?

  3. Physician presence:

Is a doctor on site throughout the entire procedure, and how many treatments are they supervising at the same time?

  4. Timing of informed consent:

When does the detailed consultation about risks and the procedure take place? Well in advance, or only on the day of treatment?

  5. Safety standards:

Are lab tests run beforehand, and how does the medical team respond to abnormal results?

  6. Aftercare:

Who can I contact after I get home, and in what language is aftercare offered?

Your timing

Reliable Planning Starts With a Clear Assessment

The right time for a hair transplant depends on the condition of your hair, not simply your age.

The donor area at the back of your head has a fixed natural supply of follicles. As hair loss progresses, more areas need coverage, changing the balance between your needs and the follicles available (Unger et al. 2013). That's why an early medical assessment of this balance makes sense.

FAQ

FAQ: Common Questions About the 5-Doctor Standard

Both, with clearly defined roles. Doctors always handle five stages of your treatment, without exception: assessing your hair before you travel, creating your personalized treatment plan based on the Pre-Test System results, planning your hairline and graft distribution, administering anesthesia and performing the final check. Medical specialists with at least seven years of experience perform the manual steps of extraction and implantation under physician supervision, following the doctor's specifications.

As co-founder of Elithair and medical director of the clinic, he is responsible for our procedures and their ongoing development. Our doctors carry out the five physician checks required by the 5-Doctor Standard; he does not always perform them personally.

Your personalized treatment plan is based on your Pre-Test System results. If they show that treatment is not advisable at this time, we do not proceed with surgery. The doctor responsible for your care makes that decision. Depending on the findings, we recommend having the issue evaluated at home and scheduling the procedure for a later date.

More than 20 doctors work at the clinic, alongside over 300 medical specialists who each focus on a single step of the process. At every stage of treatment, you'll be cared for by someone who has been performing that step for at least seven years.

On the day of treatment, we'll tell you which doctor will draw your hairline and who will supervise your procedure. Because treatment is scheduled several weeks in advance, we cannot confirm your entire team ahead of time.

No. It's included in every treatment, not sold as an add-on package. Your all-inclusive price is based on the pricing tier for the number of grafts you need. Zero Shave FUE and Manual FUE are available as optional upgrades for an additional fee.

We manage your follow-up care through the Elithair app: you upload photos there, and our medical team reviews them and supports you throughout your healing process. Every patient is guaranteed digital aftercare support.
